Belotero Balance is an FDA-approved hyaluronic acid dermal filler built for the fine, superficial lines that thicker gels struggle to treat well. Here is what sets it apart, and why your injector may select it for you. Most fillers form a defined pocket of gel that lifts a feature. What is Belotero® Balance and how does it work?

Belotero is a dermal filler made of hyaluronic acid, the same water-binding sugar your skin produces naturally and loses as you age. What makes it different from other hyaluronic acid fillers is how it is made. The Cohesive Polydensified Matrix (CPM) technology produces a single cohesive gel with low- and high-density zones inside it, rather than the uniform structure most fillers have. That variable density is why Belotero integrates so evenly into tissue and can be placed close to the surface without looking like a ridge. Once injected, the hyaluronic acid draws in water to smooth and cushion the area, with results you can see the same day. In the United States, the filler carrying this technology comes in two forms: Belotero Balance, and Belotero Balance (+), which adds lidocaine for comfort. Both are FDA-approved for moderate-to-severe facial wrinkles and folds, such as nasolabial folds, and Belotero Balance (+) also carries a specific FDA approval for the under-eye hollow.

What areas and concerns can Belotero® treat?

Belotero is at its best on softer, more superficial concerns. Commonly treated areas at Eminence include:

  • Fine lines around the mouth, sometimes called perioral or smoker's lines
  • Vertical lip lines and the border of the lips
  • Nasolabial folds, or smile lines
  • Marionette lines at the corners of the mouth
  • Superficial, crepey wrinkles that heavier fillers would over-correct
  • Under-eye hollows and shadows
If your concern is deep volume loss or building structure along the cheeks, jawline or chin, a firmer filler like Radiesse or a NASHA-based Restylane product is usually the better tool, and we carry those too.

What do Belotero® lip results look like?

Soft and natural, which is the reason people ask for it by name around the mouth. Because the gel blends into the tissue rather than forming a firm pocket, it smooths vertical lip lines and refines the lip border without the stiff, overfilled look that worries a lot of first-timers. Your lips still move and feel like your lips. Most patients see the change right away, then watch it settle over the first three to seven days as any minor swelling resolves and the filler integrates. If your goal is a dramatic size increase rather than refinement, a firmer lip filler may suit you better, and your injector will say so at the consultation.

Will Belotero® help under-eye dark circles, or only hollows?

These two concerns often get lumped together, but they are not the same problem. Belotero treats the hollow under the eye, which is the shadowed groove that forms when volume there thins out. In September 2023, the FDA approved Belotero Balance (+) specifically for the infraorbital hollow in adults over the age of 21, and filling that groove softens the shadow it casts, so many patients look brighter and less tired.  What filler cannot fix is true pigment, the brownish discoloration some people have from genetics or sun exposure, since no filler changes skin color. The under-eye is thin, unforgiving skin where a poorly placed product can throw a bluish tint called the Tyndall effect, which is exactly why Belotero's blending is prized here. At your consultation, your injector will tell you honestly whether your circles are hollow, pigment or a mix, and whether filler is the right call.

Am I a good candidate for Belotero®?

Belotero suits adults in good general health who want to smooth fine lines or refine a delicate area without adding obvious volume. It is a strong first filler for anyone nervous about looking overdone, since the whole point of the product is subtlety. It is not appropriate during pregnancy or breastfeeding, if you have a bleeding disorder, a history of severe allergic reactions, an active infection at the treatment site, or a known allergy to lidocaine in the case of Belotero Balance (+). If your facial volume shifted after significant weight loss, tell your provider, since we often coordinate care with our medical weight loss program. What happens during a Belotero® appointment at Eminence?

Plan on 15 to 45 minutes, depending on how many areas you are treating. Your injector reviews your goals, photographs the area and cleanses the skin, then applies topical numbing. Belotero Balance (+) also contains lidocaine, so the area continues to numb as treatment goes on. Filler is placed with either a fine needle or a blunt-tipped cannula. Your injector will have you sit up and make expressions partway through to check the result in motion rather than at rest.  A little prep helps: book at least two weeks before any event, and with your prescriber's approval, pause fish oil, vitamin E, anti-inflammatories and blood thinners for several days and skip alcohol for 24 hours beforehand.

How many syringes of Belotero® will I need?

This depends on how many areas you are treating and how deep the lines are, but most people refining a single area do well with one syringe, and one to two syringes cover most treatment plans. Etched lip lines or a set of under-eye hollows often take just one, while treating several areas at once naturally calls for more. Rather than guess, your injector maps out the areas at your consultation and recommends an amount, and you will know the plan and the price before anything is injected.

Does Belotero® hurt, and is there downtime?

Most people describe pressure and a brief pinch rather than pain, helped along by topical numbing and the lidocaine in Belotero Balance (+). Downtime is minimal. Expect some redness, tenderness and possible pinpoint bruising for one to three days, with the under-eye and lips being the most likely to swell.  You will see a difference the same day, though the result keeps refining over the first three to seven days as any swelling eases and the filler settles in. Skip strenuous exercise, saunas and facial massage for 24 hours, sleep with your head slightly elevated the first night and avoid pressing on the treated area. Most patients head back to work or errands the same day.

How long does Belotero® last?

Most patients see results for about six to 12 months, depending on the area and how much it moves. The softness that makes Belotero blend so well into fine lines is the same quality that lets your body metabolize it a little faster. Areas in constant motion, like around the mouth, tend toward the shorter end, while quieter areas hold longer. Many patients schedule a touch-up once a year to maintain the result.

Can I combine Belotero® with Botox® or other treatments?

Yes, and many patients do. Belotero fills and smooths static lines, the ones etched into the skin at rest, while a neurotoxin like BOTOX relaxes the muscle movement behind dynamic wrinkles such as crow's feet and forehead lines. The two address different problems, so they pair well in a single visit. Belotero also sits comfortably alongside skin treatments that work on surface texture and tone. During your Full-Face Assessment, your injector looks at the whole picture and suggests a sequence that makes sense, rather than treating one line in isolation.

How much does Belotero® cost in Princeton, NJ?

The cost of Belotero depends on how many syringes your plan calls for and how many areas you are treating, which is why we do not publish a flat price. What we can tell you is that your consultation is always free, our value-based pricing applies when you treat with one syringe or more, and membership pricing and financing are available. How does Belotero® compare to JUVÉDERM® and Restylane®?

All three are hyaluronic acid fillers, and we carry each of them, so there is no reason to push one over another. The difference is how the gel behaves and what you want to fix. Belotero is the softest and most integrating of the group, which is why it shines on fine lines, superficial wrinkles and delicate skin where a firmer product might look overdone. JUVÉDERM tends to be smooth and cohesive with more lifting capacity, a strong pick for adding volume to lips and cheeks or filling deeper folds, and it often lasts longer as a result. Restylane products span a wide range, with firmer formulas for structure and softer ones for refinement.  As a rule of thumb, reach for Belotero when the goal is blending and subtlety, and for a firmer filler when the goal is volume or lift. Because all three are hyaluronic acid-based, they share a reassuring safety net: if you ever want to reverse the result, they can be dissolved with an enzyme called hyaluronidase, usually within a day or two. Plenty of our patients use more than one, in different areas, at the same appointment.
